Our Community-Based Housing program provides autonomous, independent living solutions in subsidized rental units across the Chicagoland area for individuals and families who are referred to us via the Coordinated Entry System or via internal waitlists. We operate under the Housing First approach and believe a stable home is an essential foundation in reaching and maintaining positive health outcomes and greater well-being.
Individuals and families impacted by HIV/AIDS and other chronic illnesses who are experiencing homelessness or housing instability receive housing information services, which includes navigation of housing-related goals and resources in Chicago, and are connected to additional Chicago House services, including: case management, pharmacy, employment and legal services.
Our goal is to create and maintain permanent housing solutions while improving the health and well-being of the individuals we serve.
