Scattered-Site Housing

Scattered-site housing finds homes for homeless and disabled individuals through rent subsidy agreements with landlords. This enables Chicago House to expand our housing capacity through existing apartments.

Hospital to Housing Program (HHP)

 

Serves disabled homeless individuals coming from hospitalization, and places them in permanent supportive housing with intensive case management services. Program works to stabilize participants' health and housing using Housing First and Harm Reduction models.

The Samaritan Housing Program

 

Serves disabled chronically homeless individuals coming from hospitalization, shelters, and streets. Program works to stabilize participants’ health and housing using Housing First and Harm Reduction models.
